Employee Wellness Fitness Assessments

Once per year, employees and spouses are encouraged to conduct their own fitness assessment that includes walking, running, pushups, curl-ups and using a sit-and-reach box.

The purpose of the fitness self-assessment is to assess overall physical fitness levels by conducting cardiovascular, muscular endurance and flexibility tests. It is important to remember that you do not need to score in the excellent category on every test.

Instructions

Download the Fitness Self-Assessment Tracking Sheet, follow the testing instructions for each exercise, record your results on the tracking sheet, calculate your results using the websites listed with each exercise, and upload the completed tracking sheet to the Rewards program located in your Employee Wellness account.

We recommend that you schedule a time to reserve the push-up counter and the sit-and-reach box.

Tests may be modified or omitted to accommodate individual needs with advance notice. Contact Employee Wellness should you have any further questions concerning testing.
