In-Person assistance requires photo identification and W# to process any request.

Email requests must be sent from your @mail.weber.edu account to registration@weber.edu.  For your safety, we cannot process requests from external email accounts.  Please include your W#.

Faxed requests must include your name, W#, what you'd like us to do, and your signature. Our fax number is 801-626-6679.

Our Office will be closed on MLK Day on Jan 16 and President's Day on Feb 20, 2012.
