Insurance Providers
Weber State University General Liability, Professional Liability, Errors and Omissions, and Automobile Liability insurance is provided through the State of Utah Risk Management Fund.
University employees are insured for work related injuries or illnesses through the Worker’s Compensation Fund of Utah.
Certificate of Insurance
A certificate of insurance is a document provided to an outside organization indicating evidence of Weber State University’s insurance coverage. Outside organizations typically request a certificate of insurance when the University is holding an event or activity at an off-campus location and there is a written contract which requires insurance.
To request a certificate of insurance, contact us at least 7 business days prior to the certificates due date along with a copy of the contract so that the specific insurance requirements can be met. If a contract is not available, provide contact information and answer the following the basic questions that describe who, what, when, where, why, and how.
