Level 1 Contacts
A student with a complaint is asked to try to resolve them first with the person, office, or academic unit (e.g., a college) with whom the student has the grievance. We characterize such a complaint resolution process as occurring at the primary level or level 1 with most complaints being resolved at Level 1. This page has a list of Level 1 contacts and processes generally processes. Different Level 1 processes may include variations from the information below based on handbooks or other more specific processes created. - Medical reasons: Statement from the physician or hospital on official letterhead that provides:
- Date the medical condition was diagnosed.
- Date/Dates of treatment.
- Dates of hospitalization or confinement, recuperation period, release from treatment.
Please do not submit medical bills or insurance claims.
- Military deployment:
- Deployment papers if the student's military unit was activated and deployed during the semester.
- Other official legal documents that substantiate the inability to participate in courses during the semester due to the deployment or military commitments.
- Death in immediate family:
- Immediate family is defined as spouse, sibling, children, or parents.
- Will need to provide a certificate of death or obituary listing relationship to the deceased individual.
- Other: Special circumstances beyond the student's control may be considered.
- Documents to substantiate the claim and a letter, on official letterhead, from the source supporting the claim needs to be submitted.

Process: When a student has an issue, they first speak with the professor where the issue arose. If a satisfactory resolution is not met, the issue is then taken to the appropriate department chair. The department chair performs an assessment and submits their decision in writing. If the student disagrees with the chair’s decision, they are then referred to the associate dean who will collect as much information as possible. After the ‘information collecting phase’, the associate dean submits their decision in writing to the student.

801.626.7430Process: The petition is a one-time exception to the SAP requirements. Petitions are reviewed and approved on a case-by-case basis.
The Financial Aid & Scholarships committee will consider the SAP standard(s) you failed to meet, the type and degree of circumstances affecting the failure, the extent to which these circumstances have been resolved and your Financial Aid & Scholarships history.
A financial aid advisor will contact a student via their Wildcat Mail to acknowledge receipt of a financial aid petition and if any additional documents are required. Once a complete petition is submitted, the Financial Aid Office will have up to thirty days to make a decision. If a student does not deliver all the required documents within thirty days, the appeal will go to the Financial Aid & Scholarships Committee as is. Be aware that any supporting documentation received after the petition semester ends will not be accepted as we cannot process petitions for a previous semester.
Petitions that are denied by the Financial Aid and Scholarship Committee can be re-evaluated by the University Associate Provost upon request of the student.

For non-SEVIS related issues the student will complete a Google complaint form which is submitted to the director. This form can be found on the ISSC website. The director will have 5 working days to review the complaint and respond to the student. If the student contests the decision, they will be referred to the level 2 process. For SEVIS related complaints, the process is similar, except there is no referral to level 2. The process is completed at Level 1.

Process: This process is in connection to academic complaints related to credit bearing courses offered by the Library: Students register complaints or grade change requests with their instructor. They can then appeal that decision, or make other course-related complaints, to the Dean of the Library.

If the student meets such criteria they may fill out all necessary forms and contact Tanya Scott if they need assistance during the process.
Typical Issues Petitioned Through the Academic Petition Committee
Graduation: requests for a waiver or course substitution of the following University requirements:, back-dating a Degree, Diversity- Residency (30 hours in residency at WSU for Bachelor's degree, 20 for an Associate's Degree), Scientific Inquiry, Total Hours, and Upper Division.
General Education Requirements: requests pertaining to Breadth, Core, CIL, or all other general education requirements must go directly through the associated departments.
Major/Minor Requirements: request for exceptions to major, minor, or emphasis requirements must be submitted directly to the associated academic department.
Records: Retroactive add, cancelation, or withdrawal of a course(s), exceptions to academic records posting deadlines or processes, exceptions to academic renewal policy, and other academic records policies or procedure.
Registration: late cancelation, late withdrawal, modification of variable credits, changes in CR/NC or AU status, and exceptions to any registration policy or procedure

Process: Discuss issue directly with the decision-maker involved. In the case of academic complaints, this means the course instructor. If the student is unsatisfied with the outcome, he/she should meet with the chair of the instructor's department. If he/she remains unsatisfied, he/she should meet with the dean (or associate dean, if the dean is unavailable) for the final Level 1 decision.

801-626-7334Process: Students with a complaint would schedule a meeting with the Director of Student Involvement and Leadership (Tara), and based on the nature of the their concerns/complaint, the Director will then connect the student(s) with the appropriate advisor(s) related to the issue (i.e. student activities, WSUSA Senate, clubs and organizations, leadership programming, etc.). The Coordinators of SIL over each of these areas (the advisors) will then work with the Director of SIL and the student(s) to see if the issue(s) can be resolved.
