COVID-19 Travel Survey – Students

This communication was sent to students on March 11, 2020. 

Dear Weber State students:

In light of the COVID-19 virus, the Utah System of Higher Education (USHE) has asked Weber State University to implement travel surveys for students, faculty and staff to self-report any travel taken outside the state of Utah since February 15.

By compiling this information, the university will be able to send targeted communications and health advisories to members of our campus community based on where they’ve traveled.

Weber State is asking all students and employees to respond to this survey, including those who plan to travel (whether business or personal) through April 20. Please take a minute and fill out this survey today.

The information submitted will be kept confidential.

As of March 10, the Utah Department of Health has reported two confirmed cases of COVID-19 in the state. State health officials say the immediate health risk from COVID-19 to the general public remains low. As a result, the university continues to hold classes as scheduled and function as normal.

If you are experiencing symptoms consistent with COVID-19 (fever of 100.4°F/38°C or higher, cough or have trouble breathing)., contact your healthcare provider and stay home until you are symptom free for 24 hours. 

Contact your instructors and let them know the reason for your absence. They will work with you to stay current on your coursework.
