Application & Checklist

To enroll in the CLS 1000 course, a) you must be employed in a clinical laboratory, and b) you will need to complete the following:

1.  Fill out the Application for Enrollment in Non-Degree Programs.  Please indicate at the bottom of the form the class and semester you would like to take it.  Note that since this application is for non-degree programs, the normal $30 WSU application fee is NOT required.

2. Have your laboratory manager read and sign a Statement of Support.

The CLS department will forward your information to the WSU Continuing Education Department, who will then register you for the class and initiate a bill. 

One web site you will need to get familiar with before you begin an online class is http://wsuonline.weber.edu/. This is a great site for the how to's and what if's of distance learning.
