In order to complete the required practicums in the CLS AAS and BS degree programs, you will need to have a drug screen, fingerprinting and background check completed. 

The clinical facilities where you will be doing your practicums are going to treat you as an employee. Many of the clinical facilities require screening of their potential employees prior to being hired. Therefore, for in order for you to complete your practicums, you must comply with the rules set forth by the clinical facility. This requires that all CLT/CLS students at WSU undergo the following:

1. Drug screening
   
   A urine drug screening is required of all CLT/CLS  students at WSU. This drug screening must be completed by December 1. There are a number of clinic locations where tests can be completed in Utah.

   When you go your screening, bring the following three items:

   • Your "permission to test" document. This document identifies you as a Weber State University CLS student. If you do not have this "permission to test" document, you will not be allowed to take the drug test.

   • $30 fee for the urine drug screen. Cash or cashiers check/money order only.  No credit cards or personal checks will be accepted.

   • Photo I.D. (drivers license, passport).
   
   Please note: Drug screening times are 8 a.m. - 4:00 p.m. Monday through Friday ONLY. There is no Saturday or Sunday screening. An appointment is not necessary, but please do not arrive right at closing time.

   Results of the Urine Drug Test will be sent directly to Weber State University, Clinical Laboratory Sciences Department at 3905 University Circle, Ogden, Utah 84408-3905.

   IF YOUR DRUG TEST REVEALS THE PRESENCE OF A NON-PRESCRIBED CONTROLLED SUBSTANCE, IT MAY AFFECT YOUR ELIGIBILITY TO ENTER AS AND/OR REMAIN A STUDENT IN THE WSU CLS PROGRAM.

2. Fingerprinting

   You will need to have your fingerprints taken. There are a number of agencies in Utah where you can go to have your fingerprints taken.
