ADMISSIONS INSTRUCTIONS FOR ENTRY INTO THE
FIRST PHASE OF THE SOCIAL WORK PROGRAM
Please carefully read the following instructions!
This application must be submitted when the student is enrolled in or has previously taken Social Work 2100–Human Behavior and the Social Environment or an approved equivalent. A deadline will be announced and posted on Bulletin Boards when the applications are due. Applications should be submitted the semester prior to which the student wishes to be admitted into the Social Work Program.
A completed Social Work admissions form. Please fill out all information completely. Give additional information to clarify where needed.
A one (1) page autobiographical sketch, including reasons for choosing social work as a major. Discuss any social service related work or volunteer service. Be specific regarding hours spent in paid work or volunteer service.
A current copy of all transcripts (can be unofficial). Highlight the SW prerequisite classes on your transcript.
Two (2) Online Reference Forms submitted here http://www.weber.edu/socwkgen/studentresources/References.html. Letters from your references are optional, but accepted. The Online Reference forms are "Required". References must be from professional individuals who know you–for example: work supervisors, religious leaders, or professors. References from family or friends will NOT be accepted.
Students will be admitted to the program based on an objective point system which will include information taken from the admissions form, transcripts, autobiography, and letters of reference. All requirements listed on the admissions form must be completed prior to actual admission.
The Social Work Program Admissions Committee will review applicants each semester and select the most qualified students to fill available slots. Students will be notified by mail of their admission status.
