Step 1: General Admissions Application
Apply to Weber State University for General Admission. Applicants must submit official transcripts from all previous colleges and universities attended. The general admissions application also requires a $30 WSU application fee.
Step 2: Submit Official Transcripts
Send ALL official transcripts from all previous colleges attended to Admissions at:

Note: to ensure that your transcripts are not lost, please do not have your official transcripts sent to WSU until you have filled out your online general admissions application in step one. If you had a different name when you attended one of your previous schools, please make sure that you list this name under “Previous Last Name” during your admissions application in step one.
Step 3: Accuplacer exam for Math and English Placement
If you do not have the necessary English and or Math requirements you may be required to take an exam that will place you into the correct course based on your score. This exam is called the Accuplacer test and you must take Weber State University’s version.
Students with previous degrees from regionally accredited colleges or universities may not need to take the accuplacer.
Step 4: Required Pre-requisites
In order to be accepted into the Online AAS or BS Degree Program, you will need to have at least 3 of the following support courses completed with a C or better, and a minimum GPA of 2.5 or higher. Please note that you will need to complete all of the support courses for your degree once you are in the program. If you have already taken equivalent courses from a regionally accredited institution, we can give you credit for those courses successfully completed. All of these courses are offered online through Weber State University:
- CHEM 1110 Elementary Chemistry (Inorganic) 5 credits
- CHEM 1120 Elementary Organic/Biochemistry 5 credits
- HTHS 1110 Integrated Human Anatomy & Physiology I 4 credits
- HTHS 1111 Integrated Human Anatomy & Physiology II 4 credits
- MICR 1113 Intro to Microbiology 3 credits
- MICR 3603 Medical Microbiology 3 credits (BS program applicants only)
Step 5: Application to the Online MLT/MLS Program
Apply to the Medical Laboratory Sciences degree programs by submitting an electronic MLS Online Program Application. The application requires:
MLS Statement of Support signed by your lab manager
One-time $95 MLS application fee payable through application. This fee is non-refundable.
For BS program applicants only: Copy of MLT (ASCP) certificate. In leui of MLT ASCP certification, MT (AAB) or MT (AMT) is acceptable. Some state certifications are also acceptable.
