If you are a new student to Weber State University and need to register with Disability Services, you need to fill out the registration forms on our website. When you have completed those forms, you may email them and your documentation to firstname.lastname@example.org, or fax them to (801) 626-8990, or you can bring them into the office directly. After you have turned in the necessary documentation and forms, fill out this request form to make an appointment with an advisor in our office. The front desk will contact you to set up an intake appointment with an advisor. You are also welcome to call our front desk to make an appointment at (801) 626-6413 (voice).

Fill out this form if you need to meet with your Disability Services advisor.

Fill out this form and return to our office by email to email@example.com, in person, or by fax.
- One-Time or Short-Term Appointment Request
If you need to request interpreters or transcribers for a one-time appointment or a short-term repeating appointment, please fill out this form. This might include appointments to meet with classmates for a group project, an SI session, a meeting with a professor, or meeting with other staff and faculty on campus. These requests are for academic requests for your courses and related work in your courses.
