COVID-19 Travel Survey – Faculty and Staff
This communication was sent to faculty and staff on March 11, 2020.
Dear Faculty and Staff:
In light of the COVID-19 virus, the Utah System of Higher Education (USHE) has asked all of the USHE schools to implement travel surveys for students, faculty and staff to self-report any travel taken outside the state of Utah since Feb.15.
By compiling this information, the university will be able to send targeted communications and health advisories to members of our campus community based on where they have traveled.
Weber State is asking all employees to respond to this survey, including those who plan to travel (whether business or personal) through April 20. We will send a similar email to our students, but would encourage all of you to direct students to take the survey.
The information submitted will be kept confidential.
As of March 10, the Utah Department of Health has reported two confirmed cases of COVID-19 in the state. State health officials say the immediate health risk from COVID-19 to the general public remains low. As a result, the university continues to hold classes as scheduled and function as normal.
Contact your healthcare provider and alert your supervisor if you are experiencing symptoms consistent with COVID-19 (fever of 100.4°F/38°C or higher, cough or have trouble breathing). Stay home until you are symptom free for 24 hours.
