Classes start on Tuesday May 6, 2013 in the respiratory therapy classroom at Utah Valley Regional Medical Center at 9:00 a.m.
AAS Program
1. You can register for you courses by calling the CE registration office at (801) 626-6600 or (800) 848-7770 option #4. The office is open M-Th from 7:30 am to 5:30 pm and on Fridays from 7:30 am to 5:00 pm. You can also register in person at the CE registration office located in the Promontory Tower Bldg, Room 201 on the main WSU campus in Ogden, Utah.
2. When you speak to a CE representative, tell them your name, WSU Student ID # (or SS#), your program and the semester for which you are registering.
3. You need to pay for the courses at the time you register. If you are registering in person you may pay by cash, check or credit card. If you register by phone, you can pay by using your credit card. Any courses not paid for by the due date may incur a late fee of $40.00.
4. This program is offered as an off-campus program. The tuition will be determined by the Continuing Education Office. Because this is an off-campus program any courses taken in addition to those listed below will incur additional tuition expenses.

Pay for your CRT SAE ($35, retake $40) by October x, 2013. You may pay at the WSU cashiers office (801-626-8006, cashiers@weber.edu). Tell the cashier to apply the payment to the Respiratory Therapy department detail code M435. Ask them to send a receipt to the Respiratory Therapy Department, mail code 3904.
The CRT SAE will be at the UVU Testing Center 801-863-8544. The UVSC will charge each student an examination proctoring fee.
