Please see the Child and Family website for these times.
Outside of those times, please use the instructions below.
1) Pick up a blue background check application form and cashier’s office payment form outside Michelle .
2) Go to the Cashier’s Office and pay $40 for fingerprinting ($20 for BCI payment, $20 for Livescan office payment). Be sure you get two copies of your receipt: one for your records and one for us.
3) Bring the completed blue form and the cashier’s receipt to the advisement reception desk in Room 230 and sign up for a fingerprinting time on during one of the days listed.
4) Important: When you come for your appointment please bring your driver’s license or passport with you as picture ID. You cannot be fingerprinted without verification of identity. Clearance cards can be picked up from Michelle Checkman (room 204 for CHFam students)
5) Return to Michelle Checkman in ED Room 204 Child and Family Studies office in one week to pick up your approval card.
NOTE: Early Childhood students who plan to continue on to receive a degree in early childhood education or elementary education will need to complete another background check for the Utah State Office of Education after acceptance into the teacher education program. You may choose to complete the USOE background check at this time, if you will be done with the TE program within three (3) years.
