Please take the following steps to complete your fingerprinting process completely:

1.  Pick up the paperwork in ED room 204 from the Child & Family Studies secretary.

2. Fill out the blue application completely and return to the secretary in ED 204 with your $15 MONEY ORDER OR CASHIER CHECK made payable to:

     Utah Bureau of Criminal Identification
           (NO CASH OR PERSONAL CHECKS)

 3.  You will need to visit the fingerprinting office during their regular hours with photo ID and a CHECK OR MONEY ORDER FOR $10 made payable to:  WSU Livescan  (NO CASH)

4. Return to ED Room 204 Child and Family Studies office in one week to pick up your approval card.

NOTE: Early Childhood students who plan to continue on to receive a degree in early childhood education will need to complete another background check for the Utah State Office of Education after acceptance into the teacher education program.
