Health Risk Assessment

The health risk assessment (HRA), also referred to as a wellness questionnaire, can be completed online at your convenience.

This assessment will ask questions about your current health conditions, physical activity, diet, and mental wellbeing. At the end of the assessment, you will be given a wellness score and feedback about how your lifestyle has impacted your health and wellness. Plan on 15-20 minutes to complete the assessment.

  • How to Take the Assessment

    The Employee Wellness Office is pleased to introduce our new HRA experience. Please follow the appropriate link below to complete your yearly Health Risk Assessment.

     

    Log in using your eWeber username (the beginning of your Weber email).  If this is the first time logging in to the new HRA website, please use password: wellness1 and then you will be prompted to create your own password.

    • If you have never logged in and have not been invited by Employee Wellness to complete your HRA, please contact us at wellness@weber.edu or by calling X6480 so that login information can be created for you.
    • When participating in the Wellness Pays Bonus program, you will have 90 days to complete your Health Risk Assessment and blood lipid screening. Failure to complete both steps within this timeframe will result in a Health Risk Assessment retake. 
  • Incentives

    The screening is one step in qualifying for Wellness Pays Bonus Program and the PEHP Healthy Utah incentive.
