Please note if you choose to do assessments through more than one source, i.e. Employee Wellness, Healthy Utah, Physician's Office, you will still only qualify for rebate(s) once every 300 days.
Q: What if I completed a WSU wellness assessment between January 1, 2010 and June 30, 2010—when am I eligible for my Healthy Utah/PEHP rebate(s)?
A: If you completed a wellness assessment between January 1, 2010 and June 30, 2010, and do not want to wait until your next assessment to be eligible for your Healthy Utah/PEHP rebate(s), you may choose to have the data from this recent assessment entered into the Healthy Utah database for your “First Steps” rebate as well as your “Good For You” rebate (if you qualify). However, you may not qualify for another rebate when you are due for your assessment in 2011 as you will not be eligible until 300 days from July 1, 2010. If you choose to delay your 2011 wellness assessment to qualify for your next rebate(s), it may make you ineligible to utilize wellness time or receive incentives for participating divisions. We ask you take this into consideration as you decide when to take your first rebate(s). If you choose to wait, leaving your anniversary date the same for both rebate(s) and wellness time eligibility, you will also receive a $10 gift card to the WSU bookstore as a thank you. If you choose to proceed now, the following steps must be taken:
1. Print a consent form allowing your data to be entered in the Healthy Utah database by WSU Employee Wellness staff.
2. Sign and return the form to Employee Wellness, MC 2801.
3. Go to https://health.utah.gov/myhu/ and create a myHealthyUtah account.
4. Allow 2 weeks for Weber State to enter your data from the time we receive your consent form and an additional 2 weeks from the time Healthy Utah receives the data to activate your online Health Assessment (PWP).
5. Log in to your myHealthyUtah account https://health.utah.gov/myhu/ and complete the PWP.
6. Allow Healthy Utah/PEHP 3-4 weeks to process your rebate(s).
Q: What if I am due for my annual wellness assessment between July 1, 2010 and December 31, 2010 but do not want to wait for my anniversary date to begin the Healthy Utah/PEHP rebate process?
A: If you are due for your wellness assessment between July 1, 2010 and December 31, 2010, you may sign up for an assessment beginning July 1, 2010 if you don’t want to wait for your rebate(s) or you may wait until your anniversary date. If you wait until your anniversary date and complete the Comprehensive Assessment, you will receive a $10 gift card to the WSU Bookstore as well. You may schedule your assessment by going to http://www.weber.edu/employeewellness/Schedule_Assessment.html. Keep in mind the assessment option you select may affect your eligibility for wellness release time.
Q: What if I have never participated in WSU Employee Wellness or have in the past, but it has been longer than one year and I want to begin the Healthy Utah/PEHP rebate process?
A: If you have never participated in WSU’s Employee Wellness Program, or participated in the past, but it has been longer than one year, please sign up for an assessment by going to http://www.weber.edu/employeewellness/Schedule_Assessment.html. Keep in mind the assessment option you select may affect your eligibility for wellness release time. You will receive a $10 gift card to the WSU Bookstore if you select the Comprehensive Assessment.
Q: Can I complete an assessment through Weber State's Employee Wellness Program and Healthy Utah/PEHP each year?
A: Yes. You may choose to do this if you would like different anniversary dates for your wellness release time and your Healthy Utah/PEHP rebate(s). Please keep in mind, you will still only qualify for rebate(s) once a year. For the Healthy Utah/PEHP asssessment, you may vist their website to make an appointment.
Q: What if I prefer Healthy Utah do my assessment?
A: You may go to one of their testing sites by making an appointment at http://www.healthyutah.org/programs/testings.php. You will not qualify for wellness release time, only Healthy Utah rebate(s).
