BOOK REFLECTION FORM
After reading a wellness-related book, complete this form. You must fill out this form in order to receive credit for reading your book.
You can track your physical activity on this form. A brief note about what activity was performed and the length of time will be adequate. A minimum of 150 minutes (2.5 hours) of physical activity over at least 3 days per week, is required (unless otherwise approved by the Employee Wellness office).
STRESS MANAGEMENT LOG
You can track your personal stress management program on this form. A brief note about what activity was performed and the length of time will be adequate. A minimum of two activities focused on stress relief per week is required. Examples of acceptable activities include, but are not limited to, reading, getting a massage, visiting the Stress Relief Center (Stress Lab), taking a bubble bath, meditating, meal-planning, etc. Contact us with any questions or concerns.
