Check your blood sugar daily for twelve weeks, and record it on this form.
BLOOD PRESSURE LOG
Check your blood pressure once per week, for twelve weeks, and record it on this form.
BOOK REFLECTION FORM
After reading a wellness-related book, complete this form. You must fill out this form in order to receive credit for reading your book.
EXERCISE LOG
You can track your physical activity on this form. A brief note about what activity was performed and the length of time will be adequate. A minimum of physical activity on 3 days per week, for 30 minutes each day is required (unless otherwise approved by the Employee Wellness office).
