The applications and information packets listed below are provided for your convenience. These pages are in PDF format; to view and print copies, you will need Adobe Acrobat Reader on your computer. If you do not have it, click here and follow the instructions to download the free software.

Click on the links below for the appropriate application forms. We do not require letters of recommendation; however, three (3) reference forms are attached to the application and must be filled out by three individuals.

Application Checklist

  1. Apply to and be accepted by Weber State University.
  2. In one packet, please submit the following by January 10:
  • Advanced area application for program(s) desired. Each program requires a separate and complete application and fee.
  • Personal essay: Number 12 on the application.
     
  • Three (3) reference forms
     
  • Official or Unofficial Transcripts: Ensure that all final grades have been posted on your transcript before turning in your application.  Do not send transcripts to the general Admissions Office for Weber State University, attach them to your Advanced application.
     
  • Evidence of current ARRT certification
     
  • $25 non-refundable fee: Check or money order.  We do not accept cash or debit/credit cards.
