Applications & Checklists

The applications and information packets listed below are provided for your convenience. These pages are in PDF format; to view and print copies, you will need Adobe Acrobat Reader on your computer. If you do not have it, click here and follow the instructions to download the free software.

Click on the links below for the appropriate application forms and checklist. We do not require letters of recommendation; however, three (3) reference forms are attached to the application and must be filled out by three individuals.

Note: If you are applying to more than one program site, you must submit the proper application and fee for each location you apply to. All materials must be included for each application (photocopies will be accepted for the reference forms).

Application Checklist

Apply to and be accepted by Weber State University.
In one packet, please submit the following by January 10:

Radiology application for either WSU-Ogden, Provo, and/or Outreach locations. Each location requires a separate and complete application and fee.
Personal essay: Number 13 on the application
Three (3) reference forms
Official or Unofficial Transcripts: Ensure that all final grades have been posted on your transcript before during in your application. Do not send transcripts to the general Admissions Office for Weber State University, attach them to your Radiologic Technology application.
$25 non-refundable fee: Check or money order. We do not accept cash or debit/credit cards.
