As an applicant to any program in Radiologic Sciences, you will need to provide three references. Reference forms are included in the program applications, but you will need to make additional copies.

Note: If you are applying to more than one program site, you must submit the proper application and fee for each location you apply to. All materials must be included in each application.

Application Checklist

1. Apply to and be accepted by Weber State University.
2. In one package, please submit the following by January 10:

• Radiology application for either Ogden Campus or desired outreach location(s). Each location requires a separate and complete application and fee.
  
• Personal essay: No. 13 on application
  
• Three (3) reference forms
  
• $25 non-refundable fee: Check or money order, NO CASH!
  
• Official or Unofficial Transcripts: Do not send transcripts to general University Admissions.  Attach them to your Radiography application.
