As an applicant to any program in Radiologic Sciences, you will need to provide three references. Reference forms are included in the program applications, but you will need to make additional copies.

Application Checklist

1. Apply to and be accepted by Weber State University.
2. In one package, please submit the following by January 10:

• Advanced area application for program(s) desired. Each program requires a separate complete application and fee.
  
• Personal essay: No. 12 on application
  
• Three reference forms
  
• $20 non-refundable fee: Check or money order, NO CASH
  
• Official transcripts: Do not send transcripts to general University Admissions. Current WSU students may submit unofficial copies of their transcripts.
  
• Evidence of current ARRT certification
