Application Procedure

Application Materials

  1. The MHA application form should be word processed, typed, or clearly printed.
  2. Keep a copy of your completed application for your records.

Application Fee

Each MHA application form must be accompanied by a $30 (U.S.) application fee. Personal checks are the preferred form of payment, but money orders are also accepted.

Checks must be made payable to Weber State University. Please make sure your name is clearly written on your check. Your application will not be reviewed without payment.

Application to the MHA program and application to Weber State University are separate processes. International applicants must submit all application materials to both the MHA program and to Weber State University at least 60 days prior to their proposed date of entry to the program in order to allow for international admission requirements. U.S. citizens and permanent residents may wish to wait until an MHA decision is made before applying for admission to the university.

Application Submission

You are responsible for submitting a complete application by mail to the MHA Admissions Committee or in person to the Dean's Office in room 401 of the Marriott Allied Health Building at Weber State University.

Notification of Admissions Decision

Each application is reviewed in depth by the MHA Admissions Committee. Decisions are made only after thoughtful weighing of all the evidence provided by each candidate and with careful consideration given to the overall balance of participants sought for the MHA program. You will be notified in a timely fashion after the MHA Admissions Committee has considered your application.

Transcripts

Official transcripts of your academic record from all previous colleges or universities attended should be submitted with your application. You may request that official transcripts be sent directly to the MHA Admissions Committee or to you. You may then forward your official transcripts with your application, but they must be sent UNOPENED in their original, sealed envelope.

GMAT or GRE

Unless you already hold a professional degree, such as an MBA, MD, JD, or similar, you are required to submit scores from the Graduate Management Admission Test (GMAT) or the Graduate Record Examinations (GRE). If you are considering a dual MHA/MBA degree, you must complete the GMAT. You must also request that an official score be sent to the Ezekiel R. Dumke College of Health Professions for verification purposes. The program code for the Dumke College of Health Professions MHA Program is 2GH-VM-68 for the GMAT and 4941 for the GRE.

Your application will not be considered complete without GMAT or GRE scores and percentiles reported. If you have not received an official score report at the time you submit your application, you should report the preliminary total, quantitative, and verbal scores from the computerized test.

International Applicants

If you are an international applicant, you will need to complete additional paperwork in order to comply with university and U.S. Immigration regulations. To be admitted, you will have to meet all academic requirements stated above and demonstrate proficiency in English. If your native language is not English, you will be required to submit a score from the Test of English as a Foreign Language (TOEFL) that is not more than two years old and is 550 or higher. If you have received a bachelor's degree from English-speaking colleges or universities, you are exempted from this requirement.

You must have taken the TOEFL and received at least your unofficial scores prior to submitting your application. Official score reports must be sent from E.T.S. to Weber State University. WSU’s institutional code is 4941.

Letters of Recommendation

You are required to submit two recommendations, at least one of which should be from someone who has known you in a professional context, ideally a current or former supervisor. Recommendation forms can be downloaded here Letter of Recommendation form.doc. Recommenders are instructed to enclose their letter of recommendation in an envelope with your name on it, seal the envelope, sign across the envelope seal, and return the envelope to you OR mail them directly to the MHA Admissions Committee at the address below.

If the recommender returns the sealed recommendation envelopes to you (the applicant), DO NOT OPEN them. Return the sealed envelopes with your application. Sealed recommendation envelopes must be included as a required part of your application.

Resume

You should enclose a current resume or CV with the application form.

Optional

If there are extenuating circumstances or concerns (e.g. your academic performance as an accurate measure of your potential, unexplained gaps in your work experience, etc.) affecting your application that you feel the MBA Admissions Committee should be aware of, please elaborate on a separate piece of paper and enclose with the application.

Mailing Instructions

Use a 9" x 12" (or page-size) envelope to mail your application. Do not insert application materials into a binder or notebook of any kind. Print your name and return address on the upper left-hand corner of the envelope.
