If you have already scheduled an appointment with a counselor at the Counseling & Psychological Services Center, please print the Intake documents and CCAPS instrument below. After filling out all documents, please bring them with you to your appointment. DO NOT send them by email as it is not a secure or confidential form of communication.

Please arrive 10 minutes early to allow for processing and to ensure that you receive your full counseling session.
