Application & Checklist

Applications for the CLS program are available from the CLS office or on this site after January 1 each year: current application (PDF file).

A non-refundable $20 application fee is required and must be received with the application. You are eligible to apply upon completion of the program's pre-requisite requirements (see below). There maybe exceptions to this policy, so please see a CLS faculty advisor. Students on academic probation are not eligible to apply.

Selections are made once each year during the months of May and June for admission to the program the following fall semester. Your application must be submitted no later than May 1 to be included in the selection process. You will receive notice of your selection status by mail no later than June 30.

Please note: If you have completed your CLS A.A.S. degree on campus at WSU and are considering switching to the B.S. online program, you are required to have at least three years clinical laboratory experience to be eligible, or you have moved out of the state of Utah).

Checklist

All CLS applicants are required to:

  1. Meet with a CLS faculty advisor. This is especially important if you are transferring from any other institution.
  2. If you are not currently a WSU student, apply to WSU. You can fill out an online application to WSU.
  3. Declare 2AO - CLINICAL LABORATORY SCIENTIST APPLICANT as your major.
  4. Complete the WSU Clinical Laboratory Sciences AAS degree requirements* and/or a CLT/MLT certification. Transfer students must have CLT/MLT certification.**
  5. Submit your CLS application to the CLS department office no later than May 1 with a $20 non-refundable application fee.
  6. A complete federal background check and drug screen is required upon acceptance to the program.

*Complete all lower division CLT courses with a grade of "B-" or better and all required CLT support courses with a grade of "C-" or better prior to starting the CLS program fall semester. A minimum GPA of 2.00 is required.

**All transfer students applying to the CLS program must demonstrate CLT equivalency by providing evidence of having MLT/CLT national certification and an associate's degree in Medical Technology/Clinical Laboratory Sciences or equivalent. Transfer students demonstrating CLT equivalency will still be required to fulfill all requirements of the WSU CLS program for graduation (i.e., chemistry, microbiology and other support courses transferred for credit must be equivalent to the WSU-required support courses.).  Additionally, grades received in CLT courses at other institutions must be equivalent to grades expected of WSU CLS students.
