Here are the forms you will need if you want to make a change to your contract. Complete the substitution form if you would like to replace a class that is currently on your contract with another. Complete the addition or deletion form should you wish to add or delete a course from your contract. Remember, you must get the appropriate signature on the form before submitting it to the BIS office. The files are in pdf format.
